What causes severe toothache?
Toothache is a symptom, not a diagnosis. Finding the cause is what stops the pain returning, which is why an examination and usually an X-ray come first.
Deep decay
Decay that has reached the nerve causes throbbing pain, often worse at night or when lying down.
A cracked or broken tooth
Sharp pain on biting or releasing, sometimes with sensitivity to cold.
A dental abscess
A collection of pus at the root or in the gum, usually with constant pain, tenderness to touch and sometimes swelling.
A lost filling or crown
Exposed dentine is extremely sensitive to cold, sweet things and air.
Wisdom tooth infection
Pericoronitis around a partly erupted wisdom tooth causes pain at the back of the jaw, bad taste and difficulty opening.
Gum problems
Gum infection and food packed between teeth can produce pain that is easy to mistake for a bad tooth.
When toothache is an emergency
Call us the same day if any of the following apply:
- Pain that is not controlled by over-the-counter painkillers
- Pain that wakes you at night or keeps you from sleeping
- Swelling of the gum, cheek or jaw
- A bad taste, discharge or a raised temperature alongside the pain
- Pain after a recent extraction or filling
- A tooth broken by an accident or knock
Go straight to A&E or call 999 if swelling is closing your eye, spreading under your jaw or down your neck, or if you are having difficulty swallowing, opening your mouth or breathing. These are signs of a spreading infection that needs hospital care, not a dental chair.
What happens at your emergency appointment
- Assessment. We listen to your history, examine the tooth and surrounding tissues, and test how the nerve is responding.
- X-ray. A digital X-ray shows decay, infection at the root and bone loss that cannot be seen by eye.
- Diagnosis explained. We tell you what is causing the pain and what the realistic options are.
- Treatment to relieve the pain. Depending on the cause this may be dressing the tooth, draining an abscess, starting root canal treatment, replacing a lost filling, or removing the tooth.
- A plan for what comes next. You leave knowing what still needs doing, roughly what it costs, and when to come back.
What you can do before you get here
- Take painkillers as directed on the packet — paracetamol and ibuprofen taken together work better than either alone for dental pain, provided both are suitable for you.
- Sleep propped up on an extra pillow; lying flat increases the throbbing.
- Rinse gently with warm salty water if the gum is sore.
- Avoid very hot, very cold and sugary food and drink on that side.
- Never hold an aspirin against the gum — it burns the tissue and does not help the tooth.
- Do not wait to see whether swelling goes down on its own.
Why antibiotics on their own will not fix it
Antibiotics can slow a spreading infection, but they cannot reach inside a dead nerve or repair a broken tooth. Unless the source is treated, the pain almost always returns once the course finishes. That is the aim of every emergency visit. Most dental pain can be settled at the first appointment, either by treating the tooth or by removing it, and we will tell you honestly if a problem needs more than one visit.
What if my face is swollen?
Call us straight away. Facial swelling means infection has spread beyond the tooth and needs treating urgently. If the swelling is closing your eye, spreading down your neck, or you are struggling to swallow or breathe, go to A&E.
Is it cheaper to just take painkillers and wait?
It usually costs more. A tooth that could have been saved with a filling or root canal often ends up needing extraction, and an implant or bridge later, if the problem is left.
Can I get an emergency appointment at the weekend?
